At Eye Associates of North Florida and Seven Hills Surgery Center, we aim to make insurance straightforward and easy to navigate. Our goal is to make your visit as clear and stress-free as possible, so you know what to expect before you arrive.
What to Bring to Your Appointment
To help us process your benefits quickly, please bring:
- Your insurance card(s) (medical and vision)
- A photo ID
- Any referral or authorization, if required by your plan
We will verify your benefits and review any estimated out-of-pocket costs with you before services are provided whenever possible.

- Medicare Part B
- CHP (Capital Health Plan)
- Florida Blue (excluding Anthem and HMO Plans)
- UHC (United Health Care)
- UMR and Surest
- Champ VA / VHA of Community Care
- PHCS / US Health Group / Freedom Life — established patients only
- Harvard Pilgrim
- Tricare for Life
- EyeMed Vision — material benefits only (frames, lenses, and contact lenses)

While we work with many providers, there are some plans we may not be in network with.
If your insurance is not listed above, it does not always mean we can’t see you, but your visit may be considered out-of-network or self-pay, and different costs may apply. Please contact us ahead of scheduling your visit for clarification.
Important to Know
- Copays, Deductibles & Any Non-Covered Service Costs are due at the time of service.
- Coverage Varies by Plan – Even within the same insurance company, benefits can differ.
- Medical vs. Vision Insurance – Depending on your reason for visit, we may need to bill a different type of insurance.
- We Verify, but it’s Not a Guarantee – We do our best to verify your benefits in advance, but final coverage is determined by your insurance provider.
No Insurance? No Problem
We offer transparent self-pay options and will always discuss costs with you in advance. Self-pay visits typically range from $265 to $370, depending on the testing and/or refractive services required at the time of your exam. An estimate will be provided prior to your visit.
